1. This
permit is intended as an extension for the existing
re-entry permit for a foreigner possessing
Limited/Permanent stay permit in Indonesia;
2. It
entitles for entry to Indonesia until a specific
date has been indicated in the re-entry extension
permit based on the approval from the Indonesian
Immigration Authority in Jakarta;
3. The
issuance of this permit is subject to the approval
of the Immigration Authority. The sponsor in
Indonesia applies for the permit on behalf of the
applicant;
REQUIREMENTS:
1. Completely filled out visa
application form
2. 2
(two) recent, colored photograph (red background),
size 11/2 X 1 1/2 inches, taken not more than 6 (six)
months.
3. Valid,
perfectly conditioned passport of at least 6 (six)
months upon arrival in Indonesia
4. The
approval copy from the Indonesian Immigration
Authority in Jakarta, stating that: specific
purpose, the name of sponsor and period of the
re-entry extension in Indonesia.
5. Pay
Visa fee in Manager’s Check from Banco de Oro, Union
Bank, Bank of the Philippine Islands (BPI) payable
to the order: Embassy of the Republic of Indonesia.
