berlaku paspor akan berakhir atau
halaman paspor telah penuh.
2. Permohonan
paspor baru tidak dapat diwakilkan,
pemohon harus datang sendiri ke KBRI.
3. Mengajukan
surat permohonan yang ditujukan kepada
: Duta Besar R.I. u.p. Kepala Bidang
Konsuler;
4. Mengisi
formulir permohonan paspor.
5. Fotocopy
paspor lama dengan menunjukkan yang
asli.
6. 5
(lima) lembar foto ukuran 3,5 x 4,5 cm
dengan latar belakang merah.
7. Permohonan
paspor baru untuk anak :
- fotocopy
paspor orang tua dengan menunjukkan yang
asli;
- fotocopy
Akta Kelahiran dengan menunjukkan yang
asli;
- fotocopy
Akta Perkawinan dengan menujukan yang
asli;
- 5
(lima ) lembar foto terbaru ukuran 3,5 x
4,5 cm dengan latar belakang merah,
8. Membayar
biaya sebesar P1,100.00 dengan Manager’s
Check dari Banco de Oro (BDO), Bank of
the Philippine Islands (BPI) dan
Unionbank. Pembayaran ditujukan kepada

Embassy of the Republic of Indonesia.
