paspor baru tidak dapat diwakilkan,
pemohon harus datang sendiri ke KBRI.
3. Mengajukan
surat permohonan yang ditujukan kepada: Duta Besar R.I. u.p. Kepala Bidang
Konsuler;
4. Mengisi
formulir permohonan paspor.
5. Fotocopy
paspor lama dengan menunjukkan yang
asli.

(lima) lembar foto ukuran 3,5 x 4,5 cm
dengan latar belakang merah.
7. Permohonan
paspor baru untuk anak :
- fotocopy
paspor orang tua dengan menunjukkan yang
asli;
- fotocopy
Akta Kelahiran dengan menunjukkan yang
asli;
- fotocopy
Akta Perkawinan dengan menujukan yang
asli;
- 5
(lima ) lembar foto terbaru ukuran 3,5 x
4,5 cm dengan latar belakang merah,
8. Membayar
biaya sebesar P1,100.00 dengan Manager’s
Check dari Banco de Oro (BDO), Bank of
the Philippine Islands (BPI) dan
Union Bank. Pembayaran ditujukan kepada: Embassy of the Republic of Indonesia.

passport should also be brought for
