Mengisi formulir permohonan;

Fotocopy paspor orang tua dengan
menunjukkan yang asli;
3.
Fotocopy Akte Kelahiran dengan
menunjukkan yang asli;
4.
Fotocopy Akte Perkawinan dengan
menujukkan yang asli;
5. 3
(tiga) lembar foto anak ukuran 3,5 x 4,5
cm dengan latar belakang merah;
6. Membayar
biaya sebesar P500.00 dengan Manager’s
Check dari Banco de Oro, Bank of the
Philippine Islands (BPI) dan Unionbank.
