Segera lapor ke Kantor Polisi
terdekat.

Membuat Affidavit of Loss dari
Notary Public.

Mengajukan surat permohonan Paspor/SPLP
yang ditujukan kepada:
Duta Besar RI, u.p Kepala Bidang
Konsuler

Mengisi formulir permohonan.
-
Fotocopy paspor yang hilang atau
apabila tidak mempunyai fotocopy
paspor yang hilang, fotocopy
dokumen identitas diri lainnya a.l.
KTP/SIM/Kartu Keluarga (KK), Akta
Kelahiran.

5 (lima) lembar foto terbaru ukuran
3,5 x 4,5 cm dengan latar belakang
merah.

Membayar biaya sebesar P1,100.00 (untuk
paspor baru) atau P215.00 (untuk
Surat Perjalanan Laksana Paspor (SPLP)
dengan Manager’s check dari Banco de
Oro, Bank of the Philippine Islands
(BPI) dan Unionbank. Pembayaran
ditujukan kepada : Embassy of the
Republic of Indonesia.

Execute an Affidavit of Loss duly
legalized by a Notary Public.

Request Letter addressed to the
Ambassador copy furnished to the
Chief of Consular Affairs Section.

Fill up an application form
-
Photocopy of the old passport or
other identification documents : ID
Card/Driver’s License/Birth
Certificate;

5 (five) copies recent photo, 3,5 x
4,5 cm, red background.

A fee of P1,100.00 (passport) or
P215.00 (SPLP) in manager’s check
from Banco de Oro, Bank of the
Philippine Islands (BPI) or
Unionbank. Payable to: Embassy of
the Republic of Indonesia.
